In EMS….

Is bad enough when EMS providers do things that are just plain wrong and then try to justify it because, well because, they are EMS providers..

Being an “Ambulance Driver” gives you no special privileges other then expediently responding to calls and transporting patients. Again the expediently part means with reasonable haste and following protocols..and road, traffic, weather conditions…

Blowing a red light, driving on wrong side of the road…is not an option you have.

Period…and those whom did that to me, back when I was able to work on the truck, got and earful from me….both at the time and when we got back to the station and I drug their butts into the Managers office and shewed them out again, usually with the Manager sitting there listening and when I was done, thanking me and having me shut the door when I left…

It is a damn billboard you are driving, remember that…

and Providers hat make comments like this..

“You don’t know how it is out here unless you’re an EMS dude/gal, so you have no right to talk about it.”

I would not give the time of day, nor ever want them in my truck with me..

Go read it all….and pay attention!!

Saturday….Links and Such…

Is a bit chilly…single digits this morning…

Quiet time as I am the only one up and moving so far, late night as I taight an EMT class in town close by, actually town I grew up in for my 2nd job, since my first does not cover all the requirements my various local, State and Federal Overlords have placed on me to help fund the various charities and special interests they so like me to pay for.
MBWITW also worked late, her 2nd job which we need to keep ourselves afloat.

So I sip my coffee and cruise the internet as is my ritual.

For excitement we have large boats filled with people running into things and tipping over…bad thing that is!!!

Here we find the bizarre, in Florida, man strikes house, his, with motorcycle, his new one, and dies…Ouch…

And more good news, or not, about the “worst” economic recovery, since, well, ever!!! Bang up job our elected dillweeds are doing lead by the head dillweed in the WH!!! Change? Change them all DAMMIT!! Every last one of them!!

Two days later, the swelling returned, and the woman told doctors she had injected herself with “bath salts,” an increasingly popular stimulant drug, at a party before she initially came to the hospital.

The doctors discovered dead muscle and a necrotizing fasciitis infection that advanced up her arm before their eyes when they cut open the skin on the woman’s arm. The medical team had to amputate the woman’s arm and shoulder and perform a radical mastectomy and skin grafts to stop the infection.

Holy bugcrap!!!

Fail here, complete and total fail..that is all I am going to say about these Pundasses! Now I certainly do not proclaim to be blogger of any note at all. Especially not in these “esteemed” folks league. But I have been in EMS about 20 years now as a 1st Responder, EMT, Paramedic, Supervisor, Educator and before that spent many years working in Printing and Fabrication as a grunt, supervisor, manager and have been around the block a few times. And I find that in EMS you have two types, those that have lived and worked in both worlds and those that have worked in EMS ….. and I’ll work with the first type anytime, the second? Often sanctimonious, self important, folks.. that tend to irritate me…eff them..Lets go ahead and ridicule someone who has burst into the National Limelight for being, good and not a spouse beating, drug using, self important jerk. So for being a light, lets jump on him like the rest of the lamestream media and diss the hell out of him…arrrgghhHH!!! I hope Tebow wins it all although it is not likely…

Okay enough…lets make some breakfast…hungry and irritated now…

Another acronym in EMS!!!

PENMAN??? WTH? A super duper hero type EMS person? Hell we have plenty of them!!!

Just what we need!! More acronyms……errr I meant these!!

And those of us in the USA bitch and moan about drivers inattentiveness..I’ll settle for that, this would be really bad..

One wonders where some EMS people work and what they actually see, this article about MONA and new studies on Oxygen and Morphine in chest pain currently being discussed really says nothing new.

ASA (Aspirin) and Nitroglycerine have shown to be effective in dealing with chest pain and heart attacks, but precautions must be taken, especially with Nitro. But they work and work well…many MDs I have worked with over the years have been strong proponents of both…

This comment, when shown and discussed in our office with young, old medics, EMTs, Flight Nurses brought out a few guffaws…

Oxygen should still be given for patients with pulse oximetry saturations less than 95% or those in shock, with acute shortness of breath, or signs of heart failure — all relatively uncommon prehospital presentations. That leaves us with nitrates and aspirin.

Holy Cow BatMedic!! “uncommon presentations” come on!! How many gurgling, burbling, I can’t breathe shortness of breath CHF calls have I been on? COUNTLESS!!!

I Have a Question….

What type of Defibrillator?

Obviously an AED would not shock anything other then a heart in V-Fib or pulseless V-Tach.

Back then you could consider “pacing” an Asystole rhythm…Not that it was often successful

It appears that it was a manual defib as the Responders used a “quick look” and saw what appeared to be a

“looked like a mixture of asystole and some V-fib [ventricular fibrillation].”

Shocking it would not have changed the outcome at this point and “dead is dead” so you certainly were not going to “harm” the victim any more, not a patient, as a patient is someone you have a shot at helping.

7 minutes, not much chance of survival, pretty much nil at that point. And if CPR was not at least started immediately…… Well good night..

Looks like a case of lawyer induced ambulance chasing here….

Oh to be young again, not saying an old(kind of) broken medic like oneself could not and CAN ride a motorcycle(between bikes currently) but sure would add to the “flavor” of working active EMS.

Quick, easy to move through traffic and get places a truck just cannot!!

Personally I am thinking one of the big bore dual purpose bikes from Kawasaki, Suzuki, Honda, BMW would work perfect in such an application….

But as the article says not to many this side of the Atlantic use them..

A use for the KED!!

 

 

Yes that wonderful extrication device to be used upon crash victims….when you have plenty of time to apply it and the patient is not critical that is.

Have not “used” one in years. In fact very few medics I know have!!

But this idea for hip fractures is awesome!

As most of the time the poor victims of such injuries are scooped, placed on a long back board or picked up and placed on the cot!! Sometimes with pain meds administered before hand!!

Always pisses me off when someone asshole of a medic does not treat a persons pain. Especially an elderly persons.
